All student athletes in grades 7-12 must have a current sports physical form on file in the high school office to participate in athletics. Students will NOT be allowed to participate in Houston High School athletics, including practice, without first having a sports physical.

The Minnesota State High School League requires physicals to be done every three years, typically prior to 7th and 10th grade. On the 2nd and 3rd year of the physical, athletes must have an updated MSHSL Health Questionnaire completed and on file prior to participation. This questionnaire is available from the website (left menu bar) or from the high school office.

Please download and print the form below and take it with you to your child’s exam appointment for the physician to complete. Forms should be returned to the high school office prior to the first practice.

*Including 6th graders if they are "moved up" to play on C-team
